Sample documents

You can use the following sample documents as the basis for developing HIPAA-compliant materials. Be sure to modify them to fit your operation.

The documents are in Microsoft Word format. If you click a link but the document doesn't open, download Microsoft Word Viewer free so you can view and print the documents.

To print, click the link. The document opens in a new window. Use the menu of that window to print (click File, then Print). To return to this page, close the window.
